• Quick and convenient! No appointment needed. Come as you are; stay in your car!
• Drive-thru lane will be south to north along 2nd St. (turn left off Main Street), east side of the building.
• 12 years-old and older.
• Wear a mask.
• Wear clothing that allows for easy exposure of upper arm.
• Consent Forms must be completed in their entirety for everyone in your vehicle receiving the flu shot.
• Registration Forms must be completed in their entirety for new patients or patients last seen prior to October 2019.
• Consent and Registration Forms are available at BSMC Clinic, http://www.bsmc.org and at the Drive-Thru Clinic. They must be completed in their entirety or shots will not be administered. We encourage completing them beforehand and bringing them with you to save time.
• Insurances will be billed. Please have current insurance information with you.
• Cash and checks will be accepted for private pay patients.
• Drive-thru shot clinics are not appropriate for individuals who are uncomfortable receiving injections. Individual appointments may be scheduled by calling the BSMC Clinic.
